If you have a bona fide request to provide public assistance, fax us your request to 1 877 274 9678. Be sure to include your client’s name, their full SSN, and their employer’s name. Please provide a fax cover sheet including your purpose, clients authorization, and your return fax number.

You can also create an invoice account. Once we verify that you’re an organization that provides public assistance you will be sent an agreement and the account will be setup as a no invoice account. You will not be billed an annual fee or verification fees.
